EVENT ORGANIZATION: A&S Play Zone, DBA as A&S Party Rental
ALL PARTICIPANTS ARE PARTICIPATING AT THEIR OWN RISK.
"Section 993.08 of the Ohio Revised Code requires that riders must obey all warnings and directions regarding this ride and behave in a manner that will not cause or contribute to injury to themselves or others. Failure to comply is a misdemeanor."
I acknowledge that all activities provided by A&S Play Zone, DBA A&S Party Rental, & the respective Event and or Organization have inherent risks that could result in injury or death. I, for myself, my guests, invitees, minors in my company, or under my care or authority, sign this Waiver and Assumptions of Risk in consideration of the opportunity to use the facility, or to participate in any parties or activities at or provided by A&S Play Zone, DBA as A&S Party Rental, & the respective Event and or Organization.
I, for myself, my guests, invitees, minors in my company, or under my care or authority, knowingly accept and assume all risks both known and unknown, and agree to release, defend, indemnify, and hold harmless A&S Play Zone, DBA as A&S Party Rental, & the respective Event Organization; its parent, affiliates, subsidiaries, principals, officers, officials, agents, and employees from all claims, injuries, and/or damages in any way related to the activities provided by A&S Play Zone, DBA as A&S Party Rental, & & the respective Event and or Organization Or their employees, agents, affiliates, subsidiaries, principals, officers, officials, agents, or contractors including, but not limited to, those arising from their negligence, misconduct, errors, and/or omissions.
I, for myself, my guests, invitees, minors in my company, or under my care or authority, agree to strictly follow the safety rules of A&S Play Zone,DBA as A&S Party Rental, & the respective Event and or Organization and acknowledge failure to do so may result in expulsion from the ride and/or legal prosecution of the law.
